Considerations for General Anesthesia
- Extensive treatment needs, such as early childhood caries (ECC)
- Conditions that typically require multiple office visits, which may not be feasible for all patients, can bedone in a single visit with GA
- Children withspecial healthcare needsand/orbehavioral issueswho cannot be treated in a traditional pediatric dentistry office
- Children who haveexperienced failed or traumatic dental proceduresand refuse treatment in a conventional office
- Children withsevere anxiety
- Children withunderlying medical conditionsthat preclude in-office sedation
Circumstances in which General Anesthesia should not be used in an Outpatient Setting, include and may not be limited to patients with:
- Severe Cardiovascular Conditions
- Example: Significant arrhythmias, congestive heart failure, or recent myocardial infarction.
- High-Risk Respiratory Conditions
- Example: Severe asthma, chronic obstructive pulmonary disease (COPD), or obstructive sleep apnea (OSA).
- Morbid Obesity
- Typically defined as BMI at or above the 95th percentile for age and sex, due to the increased risk of respiratory and cardiovascular complications.
- Certain Neurological Disorders
- Example: Cerebral palsy, uncontrolled epilepsy, or conditions requiring ventilatory support post operative.
- Renal or Liver Disease
- Example: Severe kidney failure or liver impairment, where anesthesia drug metabolism may be significantly altered.
- Coagulation Disorders
- Example: Severe Hemophilia, Severe von Willebrand disease, or patients on anticoagulants without proper management.
- Musculoskeletal Disorders
- Example: Muscular dystrophy, severe scoliosis, or other conditions affecting airway management and positioning.
- Mental Health Conditions
- Uncontrolled psychiatric disorders or severe anxiety that may complicate anesthesia and post-operative recovery.
- Immunocompromised State
- Example: Post-transplant patients, those on high-dose immunosuppressants, or individuals with advanced HIV/AIDS.
- Recent Hospitalization or Surgery
- Example: Patients who have undergone major surgery recently or have been hospitalized for significant conditions.
- Other Medical Conditions Requiring Hospital Stay Post-Surgery
- Any other condition that may necessitate close post-operative monitoring or a hospital stay due to the complexity of care required.

We had a wonderful experience with Dr Joseph’s team to have my 5 year old girl’ teeth treatment done under general anesthesia. Ms Liz helped a lot with arrangements coordinations and answering general questions. Dr Joseph answered any medical questions and concerns we had before the appointment was scheduled and explained clearly on what he would do during the procedures. We were treated with class and compassion. The staff that registered us were very very helpful. Everything was done virtually, with no wait time. My two daughters were seen within 2 weeks . This is by far the best pediatric dental sedation. The doctors and anesthesiologists were very professional. Highly recommend pediatric dentistry asleep for any parents looking for a children sedation dentist.
